Advice of Absence
This form is to be completed if you suffer unavoidable disruption or misadventure during teaching weeks or mid-semester recess weeks. Please submit documentation with this form if appropriate (eg Medical certificates).

Students are to submit one complete set of documents to the Student Enquiry Service, and one set to each Division that has been affected by the absence. It is strongly recommended that certified photocopies, rather than original documents, are submitted with the Advice of Absence form. Staff at the SES can certify documents for students, however all photocopying must be done prior to submission, as no photocopying facilities are available at the SES.
All Advice of Absence forms must be submitted by the last day of classes each semester. For semester 1 2008, this will be 6 June 2008.
Special Consideration
This form is to be used for misadventure or unavoidable disruption during the final examination period only. Documentation must be attached to the request. Please read the form carefully and follow all instructions.
Special Consideration is only available for the final examination period at the end of each semester and it is administered by the Academic Program Section, Lincoln Building level 4. The APS is only open to students during the examination period. Click here for the Special Consideration form.
