Useful Student Forms
- Advice of Absense or Other Circumstances
- To be used for anything missed during the semester. Please read the form carefully. One copy goes to the division/s affected, another must go to the Student Enquiry Service.
- Appeal Against Final Grades Form
- To appeal against your final grades.
- Campus Card Replacement Form
- If you have lost your student ID card.
- Change of Personal Details
- If you need to change your name, birthdate or gender, please submit this form to SES with a certified copy of proof of the change (drivers licence, marriage/name change certificate etc).
- Concession Replacement Form
- If you have lost your transport concession sticker, you will need to apply for the replacement through Railcorp. The current cost is $21.
- Course Transfer Form

- I Expect To Complete
- To let the university know when you expect to meet the requirements of your program.
- More than 14 Credit Points
- If you wish to enrol yourself in more than 14 credit points in one semester, you will need academic approval. Once this form has been signed, please return to the Student Enquiry Service for processing and to be enroled in the units exceeding the 14 credit points.
- Postgraduate Application Form
- For direct applications to postgraduate coursework programs.
- Proxy Enrolment Form
- To authorise a proxy to complete your enrolment without you being there during the enrolment period.
- Special Consideration
- This form is to be used for misadventure or unavoidable disruption during the final examination period only. Documentation must be attached to the request. Please read the form carefully and follow all instructions. It is administered by the Academic Program Section, level 4 Lincoln building, which is open to students during the exam period.
- Status Letter Request (Domestic Students Only)
- To request a status letter for the purposes of Centrelink, banks etc, providing details such as enrolment, full/part status or confirmation of degrees taught in English. For domestic students only, international students must go through Macquarie International.
- Transcript Request Form
- To request an official academic transcript. More information available here.
- Variation to Exemptions
- Once general credit has been granted, you may be able to apply for exemptions for specific units. This needs to be approved by the academic department.
- Waiver/Application for Special Approval
- If you wish to enrol in a unit, but do not meet the prerequisites, you will need to take this form to the academic department for approval. Once form has been processed, you must enrol yourself in the unit via estudent.
- Withdrawal Without Academic and/or Financial Penalty
- To apply for a W (withdrawn) grade after the census date, or to apply for recrediting/remission of HECS or FEE-HELP debt.
